Members to contact the respective clinic directly to make an appointment.

Your co-op membership card must be presented at the clinic during each appointment. The card entitles the bearer the privilege to be extended the corporate price for taking up the preferred health screening package.

Payment to the clinic to be made directly by individual members for any health screening or other medical services rendered.
